An intake questionnaire is a structured medical form that collects a patient's history, symptoms, medications, and goals before a telehealth provider reviews the case.

What it means

A good intake does two jobs. It screens for safety: allergies, current medications, conditions that rule a treatment out (for GLP-1 drugs, a personal or family history of medullary thyroid cancer is one), and red flags that call for a live visit or an in-person exam. It also gives the provider enough information to meet the standard of care, which in asynchronous programs may be all the information they get.

Most intakes also collect identity verification and consent, and sometimes photos, vital signs, or recent lab results. The medical questions are clinical content, written and approved by the clinic's clinicians.

Why it matters to a brand operator

The intake sits in the middle of the funnel, so it is a conversion step and a clinical step at once. Each extra question can cost completions, and each missing one can cost a safe decision or a valid prescription. The best intakes branch: short for straightforward patients, longer where an answer calls for detail.

Every answer is protected health information. Intake pages need HIPAA-grade handling, vendors under a BAA, and no advertising pixels that pass answers or health-related page events to ad platforms.
